Metformin might be the most-discussed anti-aging drug that isn't actually being tested for anti-aging — at least not yet. The trial designed specifically to answer whether it extends human healthspan has been ready to go since 2016 and still hasn't enrolled a single participant. Here's the real state of the evidence behind one of longevity medicine's most talked-about off-label uses.

What Metformin Is Actually FDA-Approved For

Metformin is an oral biguanide drug the FDA originally approved on March 3, 1995 (as Glucophage), indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes.

Metformin's FDA-approved indication remains limited to type 2 diabetes. It has no FDA approval for prediabetes, weight management, PCOS, or any anti-aging or longevity purpose — use for any of those is off-label.

The Mechanistic Case for Longevity

Interest in metformin for longevity stems from a real, well-characterized mechanism: metformin inhibits mitochondrial Complex I, which raises the cellular AMP-to-ATP ratio and activates AMP-activated protein kinase (AMPK) — the same energy-sensing pathway activated by caloric restriction and exercise, both established lifespan-extending interventions in animal models.[1]

The Human Data That Started the Conversation

The finding that sparked broad scientific interest came from a 2014 UK analysis of roughly 180,000 patients, which found that type 2 diabetics treated with metformin monotherapy had lower all-cause mortality than matched non-diabetic control patients without diabetes at all (hazard ratio 0.85) — meaning treated diabetics appeared to outlive people who never had diabetes in the first place.

A 2017 systematic review and meta-analysis further found diabetics on metformin had significantly reduced all-cause mortality compared to non-diabetics, lower cancer incidence compared to non-diabetics, and reduced cardiovascular risk compared to diabetics on other treatments.[2]

That limits how directly these findings can be extrapolated to healthy, non-diabetic adults taking metformin specifically for longevity.

MILES: The Small Pilot That Came Next

To test metformin's biological effects more directly, researchers at Albert Einstein College of Medicine conducted the Metformin in Longevity Study (MILES) — a completed pilot trial that treated 16 older adults with impaired glucose tolerance with metformin (1,700 mg/day) for 6 weeks, examining whether it produced gene-expression changes associated with younger, healthier tissue.[3]

MILES found that metformin altered multiple aging-associated pathways in muscle and fat tissue, including metabolic pathways, extracellular matrix remodeling, and genes involved in DNA mismatch repair. But the study authors themselves noted it remains controversial whether metformin is protective in people who are otherwise free of disease.[3]

TAME: The Trial Designed to Actually Answer This — Still Waiting to Start

To directly test the longevity hypothesis in a large, non-diabetic human population, the same research team designed the TAME (Targeting Aging with Metformin) trial — a proposed randomized, double-blind, placebo-controlled study intended to enroll approximately 3,000 non-diabetic adults aged 65–79 across roughly 14 U.S. research centers.[4]

TAME's design is genuinely novel for a pharmaceutical trial: rather than tracking a single disease outcome, its primary endpoint tracks time to the first occurrence of any of several age-related conditions — cardiovascular events, cancer, dementia, or death — a composite endpoint developed and agreed upon with the FDA back in 2015.[4]

Here's the part worth knowing clearly: as of August 2026, TAME remains stalled without a single participant enrolled, a decade after being proposed. The barrier isn't scientific — it's entirely financial. Because metformin is an off-patent generic drug, it doesn't attract the kind of commercial pharmaceutical sponsorship that funds trials of this scale, and the trial has spent years searching for philanthropic or public funding instead.[4]

What This Means for the Current Evidence

Because TAME hasn't launched and no positive randomized trial has yet demonstrated a longevity or healthspan benefit specifically in non-diabetic humans, the current evidence for metformin as an anti-aging intervention remains genuinely preliminary: supported by animal mechanistic data, human observational associations in diabetics, and one small human pilot biomarker study — but without a completed, definitive human efficacy trial in the actual population that would use it off-label for longevity.
